On this day, __________________, 2011, we enter into contract with details of the agreement discussed below. I, Glenn Davis, _________________, and _______________________, in affiliation with _____________________ agree to pay Hey! Mr. DJ! Ent. Services the sum of __________ dollars, half of which, is to be returned with the signed contract, payable to Hey! Mr. DJ! Ent. Services. The second half is to be paid 30 days prior to your Big Day with the completed paperwork! The services rendered in conjunction with this contract are as follows: I, Glenn Davis or a representative for myself will provide music, lights and various other entertainment to the person(s) mentioned above for a period of ____ hours, from _____ to _____ on the _____ day in ___________ 2012. It is in agreement that any services not discussed and not part of the above base fee will be accommodated upon request with a charge to the above mentioned person(s)-an amount that is provided to you in the Package Detail Form. This charge may change due to accessibility and cost of the requested extras. Such charges will be discussed and the amount, if any will be added to the base fee. If damage is incurred to any of the equipment owned or rented to Hey! Mr. DJ! Entertainment Services, by anyone associated with the aforementioned persons(s) within this contract, those person(s) will be responsible for replacing or repairing damaged properties of Hey! Mr. DJ! Entertainment Services, plus an additional twenty-five percent for lost time and money. In addition, upon early termination of this contract, no money paid prior to the completion of the services discussed will be refunded in any part. If a problem arises, I may be contacted at the above contact information. Upon, signing this contract, by which binds the aforementioned persons, they are required to have a good time and enjoy the services rendered by Hey! Mr. DJ! Entertainment Services!
